One ballot per person. A minimum number of 10 completed categories required to be a valid counted ballot. RULES: This is a voluntary reader’s poll. No purchase necessary. You must be 18 years of age to vote. Voter must give his/her valid name, address and contact information. Photocopies of this ballot will not be accepted. Illegible, misspelled, or questionable entries may not be recognized or counted. You must include a company or place of business if you are voting for a person or individual. One winner will be selected at random from all ballots received to win $500.00 cash. Winner will be responsible for all taxes applicable to the total value of the prize received. Employees (and their immediate families) of Oahu Publications, Inc. and all participating sponsors are not eligible to win. Prizes are non-transferrable, not redeemable for cash and must not be used for resale. Winner to refer to prize for additional terms and restrictions. No prize substitutions will be made. Winner will be contacted after July 7, 2021. No phone calls please. Winner authorizes OPI to use their name and likeness for promotional ads at no additional compensation.
